I agree to work within the boundaries of these policies for the duration of my employment with Fortify. I acknowledge that failure to adhere to any of these policies will result in disciplinary action ranging from verbal warning, a written notation in my file and if no action to improve has been presented, termination will follow.

I have read and understood all of Fortify’s Handbook detailing policies and procedures within the company's reach. The details have been discussed with me by the undersigned Fortify representative and any questions about the content have been addressed in detail.
