The safety of both our clients and our staff are the utmost importance! We require clients to complete detailed intake forms for EVERY new service they discover. This ensures the clients and the practitioners are fully informed and prepared for the upcoming treatment.
Intake forms are emailed to the clients at the time of booking their first ____________ (insert: initial assessment, IV, injection, aesthetics consult, etc.) and are ideally completed a week before the scheduled visit.
This can be done right from the client's profile by clicking "Email" dropdown menu in the "Online Intake Forms" section, select the desired intake/consent form and it will be automatically sent and save upon completion.
All incomplete intake forms will appears as "In Progress" on the patient profile. Incomplete intakes MUST be printed and completed manually by the client in the waiting room before heading into their treatment.
