Fortify's Confidentiality Agreement
Fortify Clients openly provide us with their personal information and health history trusting that we will safe keep and use it for the sole purpose of guiding them on their individual health journeys.
Privacy of our clients personal information is an important part of providing them with quality alternative care. We are committed to collecting, using, and disclosing personal information responsibly. We strive to be as open and transparent as possible about the way we handle personal information.
All Fortify employees who come into contact with personal information are aware of the sensitive nature of the information that has been disclosed to Fortify. Fortify employees are trained in the appropriate uses and protection of confidential information. The following are the bare minimum steps taken to ensure the confidentiality and privacy of our clients information.
Only necessary information is collected about a client.
In the situation where collaborative care is necessary, we only share information with the clients consent.
-We have an interdisciplinary care form that is sent to patients to sign.
Storage, retention, and destruction of personal information is done only with the client’s consent.
We understand the importance of protecting one’s personal information and to help our client’s understand how we are doing that, we have outlined here how our office will use and disclose information.
This office will collect, use, and disclose information for the following purposes;
To deliver safe and efficient client care
To ensure continuous high quality care
To assess health needs and provide appropriate health care
To advise clients of treatment options
To enable us to contact our clients
To establish and maintain communication with our clients via email, newsletter and by phone
To offer and provide treatment, care, and services in relationship to preventative medicine
To communicate with other treating health-care providers, including specialists, family practitioners, referring physicians, and any other providers involved in the care of our client
To allow us to maintain communication and contact with our clients to distribute health-care information and to book and confirm appointments
To allow us to efficiently follow-up for treatment, care, and billing
For teaching and demonstrating purposes within the Fortify Wellness space on an anonymous basis
To comply with legal and regulatory requirements, including the delivery of patient’s charts and records to the Manitoba Naturopathic Association, in a timely fashion
To deliver anonymous charts and records to Fortify’s insurance carrier to enable the insurance company to assess liability and quantify damages, if any exist
To prepare materials for the Manitoba Naturopathic Association , complaints committee
To invoice for goods and services
To process credit card payments
To collect unpaid accounts
To assist this office to comply with all regulatory requirements
To comply with the law, existing legislation and privacy protection protocols
Fortify will not, under any circumstances, supply a client's insurer with confidential medical history if requested. In the event this kind of request is made, we will forward the information directly to our client for review, and leave the steps necessary to our client to consent and forward.
The confidentiality agreement applies to the collection, use, or disclosure of personal information. “Personal information” includes, but is not limited to, any identifiable information about an individual that relates to their personal health information, characteristics including age, gender, colour, ethnic background, family status, education, health history, family medical history and more.
Health information is personal and sensitive and its confidentiality must be protected so that individuals are not afraid to seek health care or to disclose sensitive information to health professionals;
Fortify will always provide their clients the right to access their individual charts and personal files. If the situation arises where individuals need access to their own health information to enable them to make informed decisions about health care and to request the correction of inaccurate or incomplete information about themselves, please notify your health practitioner or one of Fortify’s wellness assistants to compile your information in a timely manner.
Employees and Contractors of Fortify will not use the personal information collected during their practice in the Fortify space for any other business venture or personal practice. A request can be made to the Fortify Owners and Management for use for additional written documentation or studies, but in doing so, the clients personal information will remain anonymous and their information will strictly be used for advancing the care Fortify Practitioners can provide.
Fortify’s Confidentiality Agreement ensures the privacy anbnd confidence our clients hold with our business and the offerings we hold with our community. By signing this agreement, you agree to adhere to previous stated rules and regulations that comply with your employment at Fortify.
