Patient Labs

Requisition by our NDs:

• Refer to Dynacare for private testing (patients will pay for these labs at Dynacare )

• Must be completed Monday-Wednesday as these are the only days they run naturopathic testing

• Our NDs will receive results directly (no AROR needed)

Requisition by patient's family doctor:

• Covered by MB health (free to patient)

• Our NDs will not receive these results therefore must send AROR to request results from family doctor

Authorized Release of Records (AROR)

• Email to patient via "Online Intake Forms" on their Jane profile

• Upon completion, the AROR will automatically save to the patient chart

• Access AROR via patient chart and print (letterhead version and scale to fit on one page)

• Create a fax cover letter and fax to clinic

• Move patient name along the Trello

Receiving Labs

• Scan and upload to patient chart titled with initials and date of testing

• Change author to corresponding ND
